5/9/2023 0 Comments Ancora pain recovery shut down![]() ![]() 7 Careful investigation, including basic science and clinical research, documented the unique susceptibility of preterm infants to adverse metabolic, behavioral, and clinical responses to acute pain, sparking a revolution in pain science in the neonatal intensive care. 6 The traditional definition of pain relying on self-reported perception and emotional experience presents challenges in non-verbal populations, including but not limited to preterm infants. In this article, the panel will review the topic in detail, addressing factors related to (a) the link between pain and aberrant maturation in the developing brain (b) methods to assess pain in the NICU (c) nonpharmacologic and pharmacologic management of pain and agitation in preterm infants and (d) knowledge gaps and future research directions.Īs recently as 30 years ago, preterm infants underwent major surgical procedures without perioperative or postoperative analgesia. In April 2019, an expert panel met at the annual Neonatal Neurocritical Care Special Interest Group ( meeting to review approaches to measuring and managing pain in the NICU. 5 An important modifiable factor that has been gaining interest in the clinical and research setting is pain in the neonatal intensive care unit (NICU). 2, 3, 4 More recent studies are shifting the paradigm of brain injury in the preterm infant from a “one-hit brain injury” to overall alteration in trajectory of brain maturation. 1 Neurodevelopmental challenges in infants born preterm often follow severe intraventricular hemorrhage (IVH) and white matter injury (WMI). Despite advances in neonatal intensive care, preterm birth remains a leading cause of neurodevelopmental disability. Improved neonatal intensive care has reduced the mortality and increased the survival of preterm infants. The rate of preterm birth is rising around the world. Chronic sedation during invasive mechanical ventilation represents an ongoing challenge appropriate containment and an optimal environment should be standard when indicated, low-dose morphine infusion may be utilized cautiously and dexmedetomidine infusion may be considered as an emerging adjunct. Rapidly acting opioids (fentanyl or remifentanil) form the backbone of analgesia for moderately painful procedures. Sucrose may be utilized judiciously as an adjunctive therapy for minor painful procedures. Nonpharmacologic interventions should be consistently utilized prior to mild to moderately painful procedures. Considering benefits and risks of available treatments, units must develop a stepwise algorithm to prevent, assess, and treat pain. Both nonpharmacologic and pharmacologic approaches are used to reduce the negative impact of pain and agitation in the preterm infant, with concerns emerging over the adverse effects of analgesia and sedatives. Although many tools have been validated to assess acute pain, few methods are available to assess chronic pain or agitation (a clinical manifestation of neonatal stress). The adverse neurobiological impact of pain and stress in the preterm infant has been well documented, including neuroimaging and neurobehavioral outcomes. “It won’t be quick in my opinion, it won’t be ‘V’-shaped, it will be gradual.Preterm infants are exposed to frequent painful procedures and agitating stimuli over the many weeks of their hospitalization in the neonatal intensive care unit (NICU). “When the coronavirus is behind us, we can reengage fully,” Daly said, noting. “What I’m hoping in the baseline is we can come back safely, we listen to public health officials, we take it slow but gradual.if we do those things and we reenter safely, then I expect us to have positive growth in 2021,” Daly said.Īsked if it could take 10 or 12 years to repair the job market, as it did after the 2007-2009 financial crisis, she said, “I’m working night and day to ensure that doesn’t happen.”īut the economy can’t rev up too quickly, not as long as the virus is still loose, she said. San Francisco Fed President Mary Daly, who appeared on CNN an hour later, said the Fed’s unprecedented actions, along with nearly $3 trillion committed by the US Congress for rescue efforts, should help. That’s not the dominant view at the Fed, which has slashed interest rates to zero, bought trillions of dollars of bonds and extended credit to local governments and businesses in an effort to prevent financial markets from imploding and keep the economy from even worse devastation. “They’ll be back very soon, and next year we are going to have a phenomenal year,” he said. Interviewed by Fox News on Friday, US President Donald Trump said that the jobs will be back.

5/9/2023 0 Comments That aha momentBeing able to communicate strategy in a common model enables other stakeholders to “buy-in” to ideas and essentially become part of the guiding coalition of change agents necessary to progress. That capability to tell the story that captures digital ambition in a coherent and visual way allows executives and teams charged with change to move past the barrier of “lack of clarity” and into solving some of the downstream barriers to progress. Taken together these initiatives begin to the tell story of an organization’s digital ambition. The Digital Journey Map models out the behaviour and the relationships that need to be established across an organisation to realize the full benefits inherent in digital initiatives. We call it the Digital Journey Map and it encompasses our belief that going forward organizations will be increasingly edge-centric, data-driven, and cloud-enabled. Together with my team, we developed a model that helps establish a common language for our customers and other users to help them frame up their own digital transformation strategies. I lead a Digital Transformation Advisory Practice. Understanding, as we have established, is the key to making real change happen. And one of the most powerful ways we have learned to abstract complexity is through the models – more specifically conceptual models.Īs Peter Chan, a distinguished career scientist and known for the development of the entity-relationship model puts it, “The purpose of a conceptual model is to provide a vocabulary of terms and concepts that can be used to describe problems and/or solutions of design.” So, a model can help provide the language of digital transformation, and this is an analogy I particularly like because establishing a common language is the key to understanding. Clarity, or the ability to abstract complex ideas into simple communicable message, is at the heart of how an idea really does become an unstoppable force. Seventy percent of organizations stall in the execution of their digital strategy according to McKinsey, and one reason cited is the lack of clarity about the organisation’s digital transformation strategy. Digital transformation has never been so top of mind to organizations around the world as they seek to re-invent their business model and be disruptive first movers – and yet it is one of the most complex ideas to communicate, and the communication itself one of the key causes of failing in the ambition. This requires multiple digital initiatives to be orchestrated, with a broad assembly of stakeholders, each with their own agendas and metrics to come together and drive real change. I am talking about the kind of work that fundamentally challenges an organization’s value proposition by enabling new business models to emerge through the application of technology. I do not mean a discreet digital initiative that, in of itself, may have a tightly controlled scope with a minimum viable product clearly at the end. What is the best tool to help with this kind of problem?ĭigital transformation falls into exactly this difficult space. How easy is it to communicate and visualize that kind of idea? Not a “thing” … but a complex, interconnected web of activities that all need to deliver in an orchestrated sequence. Product management then steps in with an agile approach to sprint and iterate to MVP (Minimal Viable Product) glory.īut how easy is it for those that are seeking to plan and execute major transformational change to achieve this in reality? Ideas that may fundamentally redefine how employees work and how customers engage with the organization. They can more easily describe the anticipated features and functions, and test the propensity to buy with focus groups, trials, and other methods. ![]() If the idea is a new product or new service offering the product owner can get to a “marketecture” rapidly – with enough content to test whether the idea has potential. That moment is the point at which the idea is so clearly communicated, and the value of it so easily visualized for people that everyone “gets it.” For some, the path to this moment of enlightenment can be a relatively straight line.Ĭontextualize this into the enterprise. How does a mere idea change to an unstoppable force? There is a “moment” of realization. 5/9/2023 0 Comments Omxplayer file time loading![]() the Standard Pi Zero (without a w in its name) Summary □□□ Case 2: Your Pi doesn’t have WiFi enabled, e.g. Now reboot your Pi to see if the video plays automatically: sudo reboot Last step: Enable auto play whenever the Pi boots: sudo update-rc.d videoloop defaults Run the following commands: - Start the video loop: /etc/init.d/videoloop start - Close the video loop: /etc/init.d/videoloop stop - Re-open the video loop if it closes: /etc/init.d/videoloop repair Create the bash script - Create a file called videoloop in the /etc/init.ddirectory: sudo nano /etc/init.d/videoloop - Copypaste the content from this link (step 3) into the file and change the varible video_path to match the path to your videofile ( /boot/video.mp4) - Change the permissions of the script to allow writing and execution: sudo chmod 755 /etc/init.d/videoloop 4. ![]()

If you have never adjusted your display settings, you should see something like this: Click the Apple Icon at the top left of the screen. As you can see from the screenshot below, the warning notification has been displayed because GarageBand has a minimum width which is bigger than half the screen, you can also see that GarageBand is wider than expected:īelow I will describe in detail how to adjust the screen resolution to make this type of scenario work better, and also how I might adjust the Mosaic layouts to better suit the apps i am using. So here is the first example, I am dropping GarageBand on the Left layout to make it occupy the left half of the screen: I have setup my MacBook Pro to use the default screen resolution settings which displays very large text but does not give enough room for large apps such as GarageBand to occupy only half the screen. To provide a detailed example of the type of issue I shall demonstrate a scenario where I am trying to work with two apps (GarageBand and Safari) in order for example to view guitar chords in Safari whilst recording tracks in GarageBand. Adjust layouts to better suit the restrictions of the apps you work with. ![]() Increase the screen resolution setting on your Mac to provide more space.I shall provide detailed information about each of these below, however the options in brief are: There are several options available to minimise this issue to ensure you get the most out of Mosaic. Here is an example of the warning notification in which Finder had a layout applied which was too small for the Finder window to match: This is usually because the layout would make the window smaller than the size restriction imposed by the App. If you are working on a small display, perhaps on a MacBook, or you have your Display Settings configured to make content look large for readability, you may find that using common layouts such as Left Half and Right Half can cause Mosaic to display a warning and the window may not be resized or positioned correctly. If the developer of an app has specified that a window cannot be made smaller than a specific size, Mosaic has to adhere to this. It is important to note however that Mosaic cannot override restrictions imposed on the sizes of windows by the Apps that own them. The great strength of Mosaic is that it allows you to effortlessly tile windows in a way that suits your workflow. Its common to want to work with multiple apps visible at the same time, and being able to quickly and neatly place them side by side for example can vastly improve your workflow. Unfortunately, PARPis in preclinical in vivo models and clinical trials in Ewing sarcoma have failed to demonstrate meaningful responses. PARP inhibitors (PARPis) have emerged as an intriguing treatment strategy for patients with Ewing sarcoma, in part because EWS-FLI1 expression confers sensitivity to PARP inhibition. Combination PARPis and NAMPTis in vivo resulted in tumor regression, delayed disease progression, and increased survival.Ĭonclusions: These studies highlight the potential of these drugs as a possible therapeutic option in treating patients with Ewing sarcoma. Mechanistic drivers of the synergistic cell killing phenotype of these combined drugs included depletion of NMN and NAD +, diminished PAR activity, increased DNA damage, and apoptosis. Results: Several promising candidates emerged, including the combination of small-molecule PARP and nicotinamide phosphoribosyltransferase (NAMPT) inhibitors, a rational combination as NAMPTis block the rate-limiting enzyme in the production of nicotinamide adenine dinucleotide (NAD +), a necessary substrate of PARP. Xenograft experiments were performed to determine efficacy and in vivo mechanism. ![]() Comprehensive metabolomic and proteomic profiling was performed to better understand the mechanism underlying the synergy. To broaden the therapeutic landscape, we explored the in vitro response of Ewing sarcoma cell lines against a large collection of investigational and approved drugs to identify candidate combinations.Įxperimental Design: Drugs displaying activity as single agents were evaluated in combinatorial (matrix) format to identify highly active, synergistic drug combinations, and combinations were subsequently validated in multiple cell lines using various agents from each class. Purpose: Although many cancers are showing remarkable responses to targeted therapies, pediatric sarcomas, including Ewing sarcoma, remain recalcitrant. 5/9/2023 0 Comments Ww2 online tank controls![]() ![]() As the battle raged, the crew of a British aircraft overhead reported, “ A tank is walking up the High Street of Flers with the British army cheering behind.” The reputation of the tank was born. Still the results, while not overwhelming, were enough to satisfy the British Army commander, Sir Douglas Haig. More than a third of the tanks failed to reach their starting positions, and once the attacks started, still more broke, were delayed or became stuck, with the result that only 9 achieved their goals. Even before the attack actually began, machines began to break down. Forty-nine tanks were dispersed in small groups with the infantry divisions that made the assault. On the morning of September 15th, 1916, the British Army attacked the German trenches just outside the French village of Flers. However, despite those flaws it made a formidable weapon, and by the late summer of 1916, with progress faltering on the Somme it was time to put that weapon to use.Ī William Ivor Castle’s photograph of a British tank during World War I. With the center of each machine occupied by a loud, hot and inadequately ventilated engine, and lacking any kind of suspension, just taking a ride in one could be an unpleasant and dangerous experience. The new tanks steered with a pair of trailing wheels and required a crew of 8 – a commander, driver, 4 gunners and 2 men to assist in shifting gears. This placement, while limiting the field of fire of the guns, made it easier to fire down into trenches as the machines passed over or beside them. Rather than in the armored turrets seen today, much of the armament of these tanks was placed in armored boxes affixed to the sides of the vehicles. They were long and rhomboidal in shape with tracks encircling the body to aid in crossing deep and wide trenches. These new tanks, built to break the stalemate of trench warfare, were unlike anything in use today. George Grantham Bain Collection (1916) ///loc.pnp/ggbain.25225īy mid-1915, several groups within the British Armed Forces had come together to consider the problem and that new technology, called, in order to preserve secrecy, the “Tank”, began to take shape. However, as trenches began to stretch across northern France and the war situation devolved into a stalemate, it became clear that these cars, limited to roads as they were, would not be the breakout weapon needed to win the war. ![]() At the beginning of the war these elements were united to create the armored car. The modern armored fighting vehicle has its roots in a series of proposals bringing together armor plate, heavy weapons and increasingly powerful internal combustion engines. As the dawn broke, a single machine made its way through ranks of soldiers, passed into the churned wasteland between the lines, began to fire, and the age of the tank began. These machines, whose descendants would, in another war, become known for their lightning speed, crawled along at under four miles per hour, picking their way through a landscape of shattered tree stumps and mud. One hundred years ago in September, a small group of machines, dreamt of by Leonardo, but hitherto unseen on the field of battle, chugged along in the predawn darkness, just south of the town Flers, France, in the region of the Somme.
